FBI Special Agent Sean Archer (John Travolta) has hunted a psychotic terrorist and criminal mastermind named Castor Troy (Nicolas Cage) for the last eight years, and is consumed by revenge because Archer believes that his lovely son, Michael, was killed in a sadistic terrorist act managed by Castor Troy, six years ago.

One day, Archer has finally cornered Troy and their fight has knocked out Troy. Before fainted Troy threatens Archer that he has planted a biological bomb placed somewhere in Los Angeles that will destroy the entire city of Los Angeles and all of its inhabitants.
The FBI discover that Troy was telling the truth. All they have to do now is to find out where it is. Unfortunately the only person who knows its location is Castor's brother Pollux Troy (Alessandro Nivola), but he refuses to talk about it. The only person that Pollux will talk to is his brother Castor.

The FBI have a plan to get the information from Pollux. They sent Archer to undergo a new radical-method surgery that can cut off people's faces and place a person's face onto another person. Archer will exchange his face with Castor’s face and assume Castor’s identity and then ask Pollux about the bomb. The surgery is success so Archer goes to the prison and successfully gets the information about the bomb from Pollux.

However, Castor Troy somehow regains his consciousness and forces the doctor who operated on Archer to place Archer's face on him. So now Castor is wearing Archer’s face and assumes Archer's identity. He wants revenge on Archer for taking his face. Castor then goes to the prison as Archer and telling him that he has destroyed all documentations of the surgery and eliminated everyone who knew about the faces exchange. He takes his brother out and leaves Archer in prison.

Now, Castor Troy turns Archer's life upside down by assuming his identity. Not only is Troy ruining Archer's mission, but his personal life as well. The real Sean Archer is now forced to trust into a nightmare he never imagines.
